Bloqueo PENG vs. Bloqueo Fascia Iliaca Suprainguinal en cirugía de cadera: Estudio de cohorte

Introducción: Las fracturas de cadera representan una causa importante de morbimortalidad en pacientes adultos mayores. El control del dolor postoperatorio es fundamental para facilitar la movilización temprana y mejorar los resultados clínicos. Objetivo: Comparar la eficacia del bloqueo PENG versus el bloqueo de fascia iliaca en el control del dolor estático y durante la movilización temprana en pacientes sometidos a cirugía de cadera. Material y métodos: Estudio observacional analítico de cohorte prospectivo, realizado en el Hospital del Norte (El Alto, Bolivia) entre junio y diciembre de 2024. Se incluyeron 40 pacientes distribuidos en dos grupos: bloqueo PENG (n=20) y fascia iliaca suprainguinal (n=20). Se evaluó el dolor mediante escala numérica analógica a las 6, 12 y 24 horas. Se utilizaron pruebas estadísticas como chi-cuadrado y t de Student. Resultados: El grupo PENG presentó menor intensidad de dolor a las 6 y 12 horas posoperatorias (p 0.000). No se observaron diferencias significativas a las 24 horas en dolor estático (p 0.427). Durante la movilización, el grupo PENG mostró menor dolor (p 0.006). No se encontró diferencia en la necesidad de analgesia de rescate. Conclusiones: El bloqueo PENG proporciona mejor control del dolor estático y durante la movilización temprana, favoreciendo la rehabilitación postoperatoria sin incremento en el uso de analgesia de rescate.
